The printable sheet

A personal record of your own wellbeing. Not therapy, medical care or diagnosis, and not medical advice.

Light and dark mode. Install it like an app. Your entries stay in the browser on your own device — no account, no subscription, nothing uploaded.
What it needs, and what it does not do.
Self-Care & Wellness Planner is an offline app from ColadoSystems that runs in your web browser from a folder on your own device. It costs $24.99 once, with no subscription and no account, and includes 12 tools, sample data to explore and a PDF guide. Your records stay on your device and are never uploaded.

Everything is saved inside your browser on this device. Nothing is uploaded and there is no account, so no company holds a copy of your wellbeing records. They are not encrypted, though: anyone who can use this device and browser can open them, so keep the device locked. Download a backup regularly (Settings → Data & backup → Back up now) and keep it somewhere as safe as the records themselves.
Yes. Open index.html and it runs fully offline. You can also host the folder on any website and choose “Install app” in Chrome or Edge to get a desktop or phone icon that works offline.
Yes. The layout adapts to phones and tablets with a bottom navigation bar. Each device keeps its own data, so use Back up / Restore to move records between devices.
Go to Settings → Data & backup → Erase records. Your own details and settings are kept; every sample record is removed. Nothing is uploaded or sent anywhere when you do it.
No. It is a personal record: you write down how you slept, how you felt, what you did and what cost or gave you energy, and it adds those entries up and shows you what they say. It is not therapy, medical care, monitoring or diagnosis, it does not assess or score you, and nothing in it is medical advice. It cannot tell a hard week from something that needs help — only a person can do that. If you are worried about your health, or about yourself, speak to a doctor or another qualified professional. In an emergency, contact your local emergency service.
Nothing. Blank days are left out of every average rather than counted as zero, so a gap never drags a figure down. Habits are counted as days out of the days you planned, with last week beside them, so missing days do not wipe the weeks either side. You can fill in any past day whenever you feel like it, and the weekly review will say plainly how many days it had to work with.
There is a run length and a best run on every habit, but they are small print under the count, not the headline. The figure the app leads with is “four days out of the seven you planned, against three last week”. Nothing in the app tells you off, goes red or resets to zero because you had a bad week.
Everything is saved in this browser on this device only. Nothing is uploaded, there is no account and no subscription. It is not encrypted, so anyone who can open this browser profile on this device can read it, and so can anyone who opens a backup file you have saved. That matters more here than in most apps, because a mood and journal history is personal — keep the device itself locked, and keep backup files somewhere safe.
It puts a PIN screen over the app when it opens on this device. It hides what is on screen from someone walking past; it does not encrypt your records. There is no PIN until you set one, and if you forget it, choose “Forgot PIN?” on the lock screen and type RESET — the PIN is cleared and not a single record is deleted or changed.
No, and it will not. Meals are a note of what you ate and how it sat. Movement is what you did and for how long — a walk counts, housework counts. There are no calorie targets, no macros and no pace goals anywhere in the app.
Yes, and that is what the printed sheets are for. “Print a sheet for it” on an appointment gives one page with what you want to raise, your own averages for the four weeks before it, and what took and gave you energy, plus ruled lines for notes. It is a record you kept by hand, not a clinical measurement, and the sheet says so on it.
