Made for People living with migraine, frequent headaches or a chronic illness who want a symptom diary that does the counting — headache days, rescue-medicine days and trigger patterns — and a clear report to take to the doctor, instead of a printable log they add up by hand
A migraine tracker and symptom tracker for one person: log an attack in seconds with its length, severity, aura, symptoms, medicine and relief at two hours; a daily check-in for chronic illness with symptoms scored 0 to 10, energy in spoons, sleep and cycle day; a headache diary calendar with monthly headache days; rescue-medicine days per month; trigger patterns compared fairly with the days without; medicines and refills; appointments with your questions; and a printable doctor’s report for the last 30 or 90 days — offline, no formulas to break. Not medical advice; follow your clinician.

For migraine, frequent headaches and chronic illness: the migraine log, symptom tracker and headache diary a printable promises, plus the counting it leaves to you. Not medical advice; follow your clinician.
A printable migraine log leaves you to count headache days by hand, a spreadsheet tracker breaks when a formula is typed over, and migraine apps want an account. This opens from a double-click, works offline, keeps your health diary on your own device, and has a dark mode that stays dim.
What it needs, and what it does not do.
Migraine & Symptom Tracker is an offline app from ColadoSystems that runs in your web browser from a folder on your own device. It costs $14.99 once, with no subscription and no account, and includes 16 tools, sample data to explore and a PDF guide. Your records stay on your device and are never uploaded.

Everything is saved inside your browser on this device. Nothing is uploaded and there is no account, so no company holds a copy of your health records. They are not encrypted, though: anyone who can use this device and browser can open them, so keep the device locked. Download a backup regularly (Settings → Data & backup → Back up now) and keep it somewhere as safe as the records themselves.
Yes. Open index.html and it runs fully offline. You can also host the folder on any website and choose “Install app” in Chrome or Edge to get a desktop or phone icon that works offline.
Yes. The layout adapts to phones and tablets with a bottom navigation bar. Each device keeps its own data, so use Back up / Restore to move records between devices.
Go to Settings → Data & backup → Erase records. Every attack, check-in, medicine, trigger and appointment is removed; your settings and your name are kept. The sample diary never had a name, so nothing of it ends up on your report. To get the common triggers back, open Triggers → Your triggers and choose Add the common triggers.
No. It is a diary that counts what you write in it. It does not diagnose anything, it does not work out any clinical score, and it gives no advice about medicines or doses. Not medical advice; follow your clinician. If a headache is sudden and severe, different from your usual ones, or comes with symptoms that worry you, get medical help straight away.
A headache day is any calendar day a migraine or headache you logged was going on, from its start to its end. An attack from 10 PM to 2 AM touches two days, so it counts both; one that ends exactly at midnight does not count the next day. A day counts once however many attacks touched it. A migraine day is a headache day with at least one migraine. Symptom flares are shown in the diary but not counted as headache days.
A rescue-medicine day is any day with at least one dose of a medicine you marked as rescue, however many doses. The note is a number you choose in Settings → What you track (10 days a month to start with). When a month reaches it, the app suggests mentioning it to your doctor. It is your own number, not a medical limit; ask your doctor what is right for you.
No. It shows patterns in your own diary: how often an attack followed days you logged something, against days you did not, with the number of days behind both. A pattern is something to talk over with your doctor, never proof of a cause. Nothing is compared until there are at least 5 days with it and 5 without, and days with no check-in and no attack are left out because nothing is known about them.
Because the trigger patterns compare days with each trigger against days without it. If you only write things down when an attack happens, everything looks like a trigger. Good days are what make the comparison fair.
Yes. The daily check-in scores the symptoms you choose from 0 to 10, with energy in spoons, sleep and cycle day, and a symptom flare can be logged like an attack. The four-week grid, the weekly symptom chart and the report all work from the check-ins.
Family Medical Records keeps a whole family’s health records: conditions, vaccinations, results and insurance. Men’s Health Organizer tracks checks, readings and results over time. Migraine & Symptom Tracker is about episodes, symptoms and triggers: attacks with their length and medicine, daily symptom scores, headache days, rescue-medicine days, trigger patterns and the report for the headache doctor.
No. It is made for one person on one device, and records do not sync between devices. To move to another device, save a backup and restore it there.
